INCOME PROTECTION
SHORT TERM DISABILITY INCOME
(If applicable)

Provides a weekly cash tax-free benefit to help pay for everyday expenses (such as mortgage / rent, utilities, childcare, or groceries) if you are unable to work due to a covered disability.
Short Term Disability benefits are payable for an off-the-job accident, sickness or pregnancy. Pregnancy benefits for normal delivery up to 6 weeks; C-Section up to 8 weeks (minus the waiting period) – can be longer if medically necessary and under doctor’s care.


LONG-TERM DISABILITY INCOME
(If applicable)

Provides a monthly tax-free benefit to replace a portion of your income which can help lessen the financial impact of a covered disability. This type of insurance can help you manage your expenses when you can’t work due to a covered disability, so that you don't have to rely solely on your savings. Enrollment in this benefit is Guaranteed Issue, so you don’t have to answer any health questions to be covered.


SUPPLEMENTAL PROTECTION
HOSPITAL INDEMNITY

The Aetna Hospital Indemnity Plan pays benefits when you have a planned, or unplanned hospital stay for an illness, injury, surgery or delivering a baby. It also pays a lump-sum benefit for admission and a daily benefit for a covered hospital stay. You can use the benefits to help pay out-of-pocket medical costs or personal expenses. Provides coverage for pre-existing conditions.


CRITICAL ILLNESS INSURANCE

Available to members and their families.The Aetna Critical Illness Plan pays benefits when a doctor diagnoses you with a covered serious illness or condition like heart attack, stroke, cancer and more. You can use the benefits to help pay out-of-pocket medical costs or towards personal expenses. It provides a lump sum cash benefit to you of $5,000 to $50,000.
This cash benefit is paid directly to the insured member when you or a covered family member is diagnosed with a covered serious condition. This plan covers such conditions like heart attack, stroke, cancer, organ failure, end stage renal failure, coronary artery bypass, Alzheimer's disease, loss of sight, speech or hearing, coma, paralysis, severe burns, and/or benign brain tumor.
You can use the money to help cover your deductible or everyday expenses like utility bills, mortgage payments or rent, day care and groceries. It’s up to you.


CANCER INSURANCE

Cancer can affect anyone — and treatment can be costly. Even quality medical plans can leave you with extra expenses to pay. Costs like plan deductibles, copays for doctor visits, and extra costs for out-of-network care can add up fast. These funds can be used to pay for household expenses that may be more difficult to manage while you’re sick. MetLife hopes that this extra financial support allows you to focus more fully on yourself and your health. Provides coverage for pre-existing conditions.


ACCIDENT PLAN

The Aetna Accident Plan pays benefits when you get treatment for an accidental injury. The plan pays for a long list of covered minor and more serious injuries. You can use the benefits to help pay out-of-pocket medical costs or personal expenses. This policy helps offset loss of income due an accident. Coverage is provided for accidents that occur on or off the job.


LIFE INSURANCE
TERM LIFE INSURANCE

Available as an active full time member working 20 or more hours per week, your spouse under the age of 70, your unmarried children ages 14-19 years (to age 25 if full-time student), and handicapped children over the age of 19 are eligible for coverage.


WHOLE LIFE INSURANCE
+Long Term Care Rider

Coverage available for members, spouse, dependent chidren under age 26, and grandchildren under age 16. Cost will never go up. Death benefit will never go down.


LIFETIME BENEFIT TERM

Family Protection - Guaranteed - Plus Benefits for Long Term Care (LTC)
Members - Guaranteed Issue up to $100,000
Spouse - Guaranteed Issue up to $30,000
(Spouse can have no more than 50% of members amount)
Dependent Child – Child Term Rider for $10,000 for additional cost of $4.18 per month.
Benefits for Long Term Care
Pays death benefits in advance for home health care, assisted living, adult day care, and nursing home care.
