2 Ways To Enroll or Review Your Coverage:
Please Note: Your have a base disability plan through your union dues. We have additional coverage options available that provide $5000 per month in year two when disabled. See below.

IMPORTANT: All plan options must be completed prior to entering your payment information and submitting your enrollment. You must select “WAIVE” or “ENROLL” for every available plan option before proceeding.
RAILROAD DISABILITY & SUPPLEMENTAL SICKNESS PLAN
RR Disability pays an average of $750 per week ($3,250 per month) Months 1-12 totaling $39,000 max.
BASE PLAN (Max benefit of $45,600)
- 90 day waiting period
- Months 4-12 $400 per month
- Months 13-24 $3500 per month
- Total with RR Disability $84,600
PREMIER PLAN (Max benefit of $71,000)
- 30 day waiting period
- Months 2-12 $1,000 per month
- Months 13-24 $5,000 per month
- Total with RR Disability $110,000
ACCIDENT PLAN
Includes a $300 per day Sickness- Hospital Confinement Benefit Rider
• Provides 24-hour coverage for accidents, on OR off the job.
• $400 Emergency Room benefit / $600 Ground Ambulance.
• $3,000 for hospital admission plus $750 per day hospital confinement.
• Additional benefits for X-Ray, CAT scan, surgery, rehab, AD&D.
• $100 Wellness Benefit

No Pre-Existing Condition Clause

CANCER INSURANCE
• 2 plans to choose from. Base Plan & Premier Plan Include following benefits & more.
• Pre-existing conditions covered after 12 months.
• Provide a lump sum initial diagnosis benefit of $5,000.
• Pay up to $2,000 per month for self-administered drugs.
• Provide coverage for 32 other specified diseases in addition to cancer.
• PREMIER PLAN provides up to $5,000 per month for chemo & radiation therapy.
• BASE PLAN provides up to $500 per day for chemo & radiation therapy

TERM LIFE INSURANCE
- Boston Mutual’s Group Term Life and Accidental Death & Dismemberment Insurance is a low cost option beneficial for employees in their early to mid working years. This plan allows you to purchase up to $100,000 up to age 70 at a guaranteed issue basis while actively employed.
- Spouse coverage is available at 50% of employee amount up to $15,000.
- $10,000 child dependent rider is available.
LIFETIME BENEFIT TERM
- Family Protection - Guaranteed - Plus Benefits for Long Term Care (LTC)
- Members - Guaranteed Issue up to $100,000
- Spouse - Guaranteed Issue up to $30,000 (Spouse can only select up to 50% of members amount)
- Dependent Child – Child Term Rider for $10,000 for additional cost.
- Benefits for Long Term Care: Pays death benefits in advance for home health care, assisted living, adult day care, and nursing home care.
