INCOME PROTECTION
SHORT TERM DISABILITY INCOME
• Guaranteed Issue for members! Includes 3/12 Pre-existing Condition Limitation.
• Pre-existing conditions are covered after 12 months from policy date.
• Provides replacement income when you are disabled from an off the job accident or illness.
• Benefits are payable for up to 25 weeks after a seven-day elimination period.
• Employees select a benefit amount from $100/week to a maximum of 80% of earnings.
• Pays in addition to your employer weekly benefit not to exceed a total of 80% of earnings.
LONG-TERM DISABILITY INCOME
• Guaranteed Issue for members! Plan includes 3/12 Pre-existing (Pre-X) Condition Limitation.
• Tax-free benefits begin after 6 months due to an on OR off the job accident or illness.
• Member can purchase up to $6,000 a month in coverage not to exceed 65% of base income.
• Tax-free benefits are payable for up to 5 years – also a 2 year LTD is available.
SUPPLEMENTAL PROTECTION
HOSPITAL INDEMNITY
The Chubb Hospital Indemnity Plan pays benefits when you have a planned, or unplanned hospital stay for an illness, injury, surgery or delivering a baby. It also pays a lump-sum benefit for admission and a daily benefit for a covered hospital stay. You can use the benefits to help pay out-of-pocket medical costs or personal expenses. Provides coverage for pre-existing conditions. $50 Wellness Benefit
CRITICAL ILLNESS INSURANCE
Available to members and their families. The Chubb Critical Illness Plan pays benefits when a doctor diagnoses you with a covered serious illness or condition like heart attack, stroke, cancer and more. You can use the benefits to help pay out-of-pocket medical costs or towards personal expenses. It provides a lump sum cash benefit to you of $5,000 to $50,000. $100 Wellness Benefit
CANCER INSURANCE
• 2 plans to choose from. Base Plan & Premier Plan Include following benefits & more.
• Pre-existing conditions covered after 12 months.
• Provide a lump sum initial diagnosis benefit of $5,000.
• Pay up to $2,000 per month for self-administered drugs.
• Provide coverage for 32 other specified diseases in addition to cancer.
• PREMIER PLAN provides up to $5,000 per month for chemo & radiation therapy.
• BASE PLAN provides up to $500 per day for chemo & radiation therapy.
ACCIDENT PLAN
• Provides 24-hour coverage for accidents, on OR off the job.
• $400 Emergency Room benefit / $600 Ground Ambulance.
• $3,000 for hospital admission plus $750 per day hospital confinement.
• Sickness Rider: $300 pr day, up to 90 days per calendar year for a covered illness. .
• Up to $100,000 accidental death benefit.

TERM LIFE INSURANCE
Available as an active full time member working 20 or more hours per week, your spouse under the age of 70, your unmarried children ages 14-19 years (to age 25 if full-time student), and handicapped children over the age of 19 are eligible for coverage.
